Job summary

Qualifications
- Graduate of an accredited Physical Therapy program with a valid state license.
- Direct patient care experience and ability to assess medical conditions and rehabilitation potential.
- Experience documenting in an EMR; PCC experience preferred but not required.
- CPR certification required.
Responsibilities
- Perform PT assessments for referred patients to determine rehabilitation needs.
- Deliver direct patient care and develop individualized treatment plans.
- Document sessions accurately in PCC EMR.
- Participate in onboarding and ongoing professional development.
