Qualifications

• Graduate of an accredited university with a PT degree (DPT/PhD optional).
• Holds current license in the state where practicing.
• APTA recognition is preferred.

Responsibilities

• Provide physical therapy evaluations based on physician orders.
• Develop and implement comprehensive treatment plans with goals, frequency, and modalities.
• Document findings, progress notes, and discharge summaries.
• Ensure MD orders are followed for evaluations, treatments and re-certifications.
• Coordinate care with the interdisciplinary team and participate in meetings.
