Phoenix Physical Therapy in Hamburg, PA is seeking a licensed Physical Therapist to join our team. You will evaluate patients, design and implement PT programs, and collaborate with an interdisciplinary care team to restore function and decrease pain.

You’ll educate patients and families, maintain documentation, and ensure safety and compliance with regulations. This full-time on-site role emphasizes patient-centered care.

Qualifications

• Graduate of an approved school of Physical Therapy.
• Licensed Physical Therapist in the state.
• Current CPR certification required.
• Proficient in PT evaluation, assessment and care planning.

Responsabilités

• Meet with patients to collect medical history and assess function.
• Develop and implement rehabilitative treatment plans.
• Coordinate care with an interdisciplinary team.
