Job summary

OhioHealth seeks a licensed Physical Therapist to provide direct patient care at Marion Medical Center. You will evaluate, treat, and reassess patients, coordinate discharge planning, and educate patients and families.

This role requires a valid PT license, and a state driver’s license if in Home Care, with collaboration across a multidisciplinary team. Day shifts with potential to work at other care sites; join a team focused on safety, quality, and patient outcomes within a high reliability

Qualifications

• Bachelor's Degree: Physical Therapy (Required).
• Driver's License only if in a Home Care department.
• Licensed in Field - State of Ohio.
• Physical Therapist License - American Board of Physical Therapy Specialties.

Responsibilities

• Evaluates and treats patients after provider referral, develops and implements plan of care.
• Reassesses and adjusts plan of care as needed.
• Maintains productivity within department expectations.
• Coordinates discharge plans with multidisciplinary team and informs physicians, nurses, and care managers.
