Job summary

Life Care Centers of America is seeking a licensed Physical Therapist to provide direct rehabilitation services to patients referred by physicians. The PT will follow established therapy plans, establish realistic goals, and work with PTAs and rehab staff to evaluate and adjust care plans.

Strong documentation and teamwork are essential. Qualified candidates hold BSPT/MSPT/DPT from an accredited program, possess an active license, and maintain CPR certification.

Qualifications

• Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT).
• Current license to practice in the applicable state and maintain active licensure.
• CPR certification upon hire or obtained during orientation; CPR must remain current.

Responsibilities

• Follow and implement physical therapy treatment plans for patients.
• Establish, assess, and modify functional goals.
• Oversee and evaluate care provided by PT assistants and students.
• Document patient progress accurately and timely.
• Collaborate on interdisciplinary team to ensure quality patient care.
• Maintain professional interactions and comply with confidentiality.
