Job summary

DayToDay Health is seeking a licensed physical therapist to provide outpatient evaluations and individualized treatment plans. You will collaborate with physicians and interdisciplinary teams to ensure high-quality, patient-centered care and contribute to continuous quality improvement.

You will document patient progress, adjust interventions based on response, and mentor student clinicians during clinical development, supporting staff education and ongoing professional growth.

Qualifications

• Licensed/credentialed physical therapist with strong clinical assessment and treatment planning skills.
• Excellent communication and collaboration skills with patients and multidisciplinary teams.
• Strong documentation skills with a commitment to evidence-based practice and quality improvement.
• Experience supporting education, training, or mentoring of staff or students is preferred.

Responsibilities

• Conduct physical therapy evaluations and develop individualized functional treatment plans.
• Provide outpatient physical therapy interventions and adjust care based on patient response.
• Maintain accurate clinical documentation of progress and outcomes.
• Work closely with physicians and care teams to support patient-centered care.
• Contribute to quality improvement initiatives.
