Job summary

Northwoods Rehab is seeking an Occupational Therapist to assess and treat patients, develop and implement individualized therapy plans, and document outcomes under physician orders.

Responsibilities include patient evaluations, treatment planning, coordinating with the care team, discharge planning, and supervision of COTAs and techs. Current state license or eligibility is required, with ongoing professional development encouraged.

Qualifications

• Bachelor of Science in Occupational Therapy or higher from an accredited program.
• Successful completion of National Certification Examination for OTR.
• Current state licensure or licensure eligibility.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provides and directs occupational therapy services to patients under physician orders.
• Conducts screenings and evaluations to inform treatment planning.
• Develops individualized treatment plans with short and long-term goals.
• Coordinates treatment with patient, family and rehab team.
• Monitors patient response and adjusts treatment as needed.
• Plans for home or community programming to maintain gains.
• Documents assessment results, treatment progress and termination of services.
• Supervises COTAs, rehab techs, students and volunteers.
