Job summary

Northwoods Rehab in Minnesota seeks a licensed Occupational Therapist to deliver comprehensive OT services to residents and collaborate with the therapy team. You will plan and implement treatment programs, document progress, and participate in discharge planning and continuing education.

This role reports to the Director of Therapy/Rehab and may supervise COTAs and rehab staff while maintaining standards of practice. Travel to home assessments may be required within a reasonable distance.

Qualifications

• Bachelor of Science in Occupational Therapy from an accredited program.
• Successful completion of NBCOT exam.
• Current state licensure or licensure eligible.

Responsibilities

• Provides and directs occupational therapy services to patients including assessment, treatment, program planning and implementation.
• Conducts screenings of patients to determine need for intervention and treatment.
• Develops treatment plans with short and long-term goals and coordinates with patient and family.
