Job summary

Lincoln Village Healthcare Center seeks an LPN to provide nursing care under RN supervision. You will administer medications, monitor health status, assist with ADLs, and support the care plan to promote resident well-being.

Responsibilities include documentation, team communication, infection control, and supervising CNAs. A valid IL LPN license and LTC experience are preferred; 12-hour shifts are available in Lincoln, IL.

Qualifications

• Current, valid Illinois LPN license in good standing.
• Graduation from an accredited practical nursing program.
• Experience in long-term care or similar healthcare setting preferred.
• Strong clinical skills and ability to respond quickly in emergencies.
• Excellent communication, leadership, and organizational skills.

Responsibilities

• Administer medications and treatments as prescribed, in accordance with policies and state regulations.
• Monitor residents' health status, observe for changes, and report findings promptly to RN/physician.
• Assist residents with activities of daily living (ADLs) as needed.
• Ensure residents receive appropriate nutritional support and hydration.
• Accurately document all care, medications, and observations in the medical record.
• Participate in care plan development and interdisciplinary team meetings.
• Communicate effectively with residents, families and healthcare team members.
• Follow infection control procedures and use PPE appropriately.
• Maintain a safe environment and comply with HIPAA and facility regulations.
• Direct and monitor CNAs, provide training and onboarding as needed.
