Job summary

Lincoln Village Healthcare Center in Lincoln, IL is seeking a Licensed Practical Nurse (LPN) to provide nursing care to residents under the supervision of an RN and in accordance with facility standards. The LPN administers medications, performs treatments, monitors residents’ health status, and supports the overall care plan to promote optimal health outcomes.

The role requires strong clinical skills and leadership to supervise CNAs. This position is full-time with 12-hour shifts (6-6).

Qualifications

Current Illinois LPN license in good standing.
Graduate from an accredited practical nursing program.
Experience in long-term care or similar healthcare setting preferred.
Strong clinical and communication skills.

Responsibilities

Administer medications and treatments as prescribed, in accordance with facility policies and state regulations.
Monitor residents’ health status, observe for changes, and report findings to the RN or physician.
Assist residents with activities of daily living (ADLs) as needed.
Document all care provided, medications administered, and resident observations in the medical record; communicate with families and team.
Supervise CNAs and participate in onboarding and orientation of new staff.
