Hartford HealthCare's Charlotte Hungerford Hospital is seeking a licensed mental health professional to perform emergency psychiatric assessments and coordinate discharge planning. Required qualifications include a Master’s degree in Social Work, Counseling, or Marriage and Family Therapy, a CT license (LCSW/LMFT/LPC), and at least two years in a psychiatric setting; four years in ED/crisis is preferred.

• Master’s degree in Social Work, Counseling, or Marriage and Family Therapy.
• CT license required: LCSW, LMFT, or LPC.
• Minimum two years' experience in a psychiatric setting; four years preferred.

• Ability to assess problems and make timely decisions.

Mansioni

• Conducts emergency psychiatric assessments and gathers collateral information for disposition.
• Advocacy, referral and discharge planning to meet patient/family needs; coordinate with providers.
• Facilitates treatment referrals and crisis intervention services as needed.
• Coordinates care and dispositions with ED staff to ensure integrated services.
• Documents assessments and all case management activities.
• Initiates treatment plans for longer-stay patients with the team.
