Job summary

Heritage Manor Care seeks a dedicated Occupational Therapy Assistant to join our post-acute care team. You will provide therapy services under the supervision of an OTR and contribute to patient progress through the Plan of Care.

Responsibilities include accurate daily notes, discharge summaries, and collaboration with physicians, families, and staff. LA-area facility offers opportunities for professional development and teamwork-focused care.

Qualifications

• Minimum of Associate of Arts in Occupational Therapy Assistant.
• Licensure or certification as required by state guidelines.
• CPR certification required.
• 1 year Occupational Therapy Assistant experience preferred.
• 1-2 years in residential/assisted living or skilled nursing facility preferred.

Responsibilities

• Provides therapy services established by the Occupational Therapist in the patient Plan of Care.
• Provides therapy services under supervision of an OTR.
• Completes documentation including daily notes and assists with progress notes, discharge summaries, or monthly reports.
• Submits billing, payroll, and expense documentation in a timely manner.
• Coordinates treatment goals with other team members.
• Attends required meetings and continues education to stay current with clinical skills.
