Job summary

Heritage Manor Care in California is seeking an Activities Director to lead and enrich the life of residents through engaging programs. You will plan, implement, and evaluate activity initiatives across the facility, coordinating with clinical staff and families.

The role involves interviewing residents and families, organizing transport when needed, and guiding discharge planning with activity care plans. Must have a high school diploma and at least 6 months of experience in a related setting.

Qualifications

• Must possess, as a minimum, a high school diploma or its equivalent
• Must have, as a minimum, 6 months experience

Responsibilities

• Plan, develop, organize, implement, evaluate and direct the activity programs for the facility
• Involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances
• Review department policies and procedures and participate in making recommended changes
• Participate in community planning related to the interests of the facility and the services and needs of the resident and family
• Participate in discharge planning, development and implementation of activity care plans and resident assessments
• Interview resident/families as necessary and in private setting
• Assist in arranging transportation to other facilities when necessary
• Provide information to resident/families as to Medicare/Medicaid eligibility, and other financial assistance programs available to the resident
• Assume the authority, responsibility, and accountability of directing the activity department
• Maintain a productive working relationship with the medical profession and other health related facilities and organizations
• Coordinate activities with other departments as necessary
• Make written and oral reports/recommendations to the Administrator concerning the operation of the activity department
• Be able to assist and coordinate family visits
