Job summary

Volunteers of America–Greater New York is seeking a Case Manager to support clients within the Safe Dwelling II program, conducting intake and assessment, developing service plans, linking to services, counseling, and monitoring progress.

Minimum qualifications include an Associate's degree or 60 college credits in Human Services, or a High School Diploma/GED with two years shelter experience; strong verbal and written communication and basic computer skills; domestic violence experience

Qualifications

• Associate's degree or 60 college credits in Human Services or HS diploma with 2 years shelter experience.
• Demonstrated verbal and written communication skills; basic computer skills.
• Domestic violence experience preferred.

Responsibilities

• Manage client intake and assessment to orient the client to the program and produce a comprehensive assessment of client service needs.
• Develop mutually agreed-upon individual service plans with the client and update as required.
• Meet regularly with the client to assist in obtaining needed services/benefits and provide supportive counseling.
• Document case management activities per agency and funder guidelines and participate in evaluation.
