Job summary

Palm Beach Nursing and Heartland Nursing and Rehabilitation Center are seeking an experienced RN/LPN MDS Coordinator to oversee MDS and care planning services across both facilities, traveling between Lake Worth and Boynton Beach as needed.

The ideal candidate has strong MDS/RAI knowledge, excellent organizational skills, and a commitment to ensuring accurate assessments, regulatory compliance, and quality resident care.

Qualifications

• Current Florida RN or LPN license.
• 2+ years of long-term care or skilled nursing experience preferred.
• Strong knowledge of RAI/MDS regulations, assessments and reimbursement processes.

Responsibilities

• Coordinate MDS assessments and submissions in accordance with federal/state regulations.
• Maintain accurate MDS, RAI, and care planning documentation.
• Collaborate with interdisciplinary team to develop resident-centered care plans.
• Monitor Quality Measures (QM) and Quality Indicators (QI) and assist with corrective actions.
• Travel between Lake Worth and Boynton Beach facilities to provide MDS coordination.
