The role involves psychosocial assessments, integrated summaries, and developing treatment plans in collaboration with a multidisciplinary team. Qualifications

• Bachelor's degree required.
• Licensure in LADC I/II, CADC I/II, LSW, or LMHC required.

Responsibilities

• Review assessments and develop an integrated summary for treatment planning.
• Develop comprehensive treatment plans with measurable goals.
• Lead/co-facilitate group therapy sessions with a focus on collaborative care.
• Maintain clinical records in accordance with licensing and program policies.
• Provide individual counseling and family therapy as scheduled.
• Coordinate with treatment team and referral sources to ensure continuity of care.
