Merakey seeks a dedicated Case Manager to join the Dual Diagnosis Treatment Team in Dubois, PA. This full-time role involves assessing, planning, and coordinating recovery treatments for adults and youth with mental health needs, including autism spectrum considerations.

Qualifications

• Bachelor's degree in Social Work, Psychology, or another human service-related field required.
• 1 year of experience in the behavioral health field required. Candidate must be open to community based travel.
• Must complete clearances and background checks at Merakey's expense.
• Valid PA driver's license with a clean driving record required.

Responsibilities

• Assess, plan, and coordinate recovery treatment and support plans for adults and youth with mental health needs.
• Provide intensive therapeutic services in settings including community living arrangements, schools, and community settings.
• Collaborate with the treatment team to implement and transfer skills from the recovery plan.
