Merakey is seeking a full-time Case Manager to join the Dual Diagnosis Treatment Team in Huntingdon, PA. You will assess, plan, and support recovery for adults and youth with mental health diagnoses.

This role involves delivering services across settings and collaborating with the treatment team to transfer skills in a person-centered approach. The schedule is M-F with on-call rotations.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Criminal Justice, or similar human services.
• Experience in the behavioral health field with mental health or developmental disabilities.
• Must have or be able to obtain required clearances and background checks.

Responsibilities

• Assess, plan, and support recovery treatment and plans for adults and youth.
• Provide intensive therapeutic services in community settings and schools.
• Collaborate with treatment team to implement person-centered plans.
