Job summary

Mindful Mobile Medicine seeks a dual-certified Family Nurse Practitioner and AGNP/PMHNP to provide comprehensive geriatric care inside senior living communities. You will partner with families, caregivers, and facility teams to deliver whole-person care, including behavioral health support, in a mobile, relationship-based model.

You will work with an interdisciplinary team to ensure high-quality, evidence-based care while promoting aging in place and minimizing unnecessary hospital visits.

Qualifications

• Dual FNP/AGNP-PMHNP certification required.
• Active Utah APRN license with prescriptive authority.
• Experience caring for older adults, preferably in geriatrics or hospice.
• Ability to practice independently while knowing when to consult.
• Strong clinical judgment and excellent communication skills.
• Focus on medication management and deprescribing when appropriate.

Responsibilities

• Provide comprehensive primary care to residents in senior living communities.
• Perform new patient evaluations, wellness visits, chronic disease management, and acute visits.
• Diagnose and manage geriatric conditions and multimorbidity.
• Evaluate and manage depression, anxiety, sleep concerns, and psychiatric symptoms.
• Coordinate care with families, facility staff, and other specialists.
• Document thoroughly and promote evidence-based geriatric care.
