Job summary

Mindful Mobile Medicine seeks a dual-certified FNP/AGNP to deliver primary and behavioral health care inside senior living communities in Utah. You will build longitudinal relationships with residents, collaborate with families, staff, and hospice teams, and manage medications with a focus on aging in place.

The role offers flexible full- or part-time schedules, autonomy with clinical support, and opportunities to practice both primary care and psychiatric care in a mobile, interdisciplinary

Qualifications

• Active Utah APRN license and prescriptive authority.
• Dual FNP/AGNP/PMHNP certification for role.
• Experience caring for older adults, preferably geriatrics or memory care.
• Ability to work independently with collaboration when needed.

Responsibilities

• Provide comprehensive primary care to residents in senior living communities.
• Conduct wellness visits, chronic disease management, and acute visits.
• Address physical and behavioral health needs with integrated care.
• Coordinate care with families, nursing staff, hospice teams, and specialists.
• Promote aging-in-place and reduce preventable hospitalizations.
• Complete timely clinical documentation and quality measures.
• Participate in interdisciplinary care planning and goals of care.
• Support palliative and hospice care referrals when indicated.
