EYBAN PCA PROVIDER LLC is seeking a compassionate Direct Support Professional to join our team in ST. Paul, Minnesota. You will provide person-centered care, assist with daily living activities, and support clients in community participation.

Qualifications

• Must complete required orientation and training.
• Must be reliable, patient, and respectful.
• Must have good communication and documentation skills.
• Must be able to support clients with daily living activities and promote independence.
• Must follow client support plans, confidentiality rules, and company policies.
• Previous caregiving experience is preferred but not required.

Responsibilities

• Provide person-centered support while respecting each client's dignity, rights, and choices.
• Assist clients with daily living activities, including cooking, cleaning, laundry, and personal care as needed.
• Encourage independence and help clients develop daily living and life skills.
• Support clients in participating in community activities, appointments, shopping, and social events.
• Follow individual support plans and provide services according to client needs.
• Monitor client health and safety and report any concerns or changes to the supervisor.
• Provide medication assistance as authorized and in accordance with training and applicable policies.
• Maintain accurate daily documentation and report incidents as required.
• Protect client confidentiality and maintain professional boundaries.
• Promote a safe, respectful, and supportive living environment.
• Communicate effectively with supervisors and other members of the support team.
• Follow all applicable DHS regulations, company policies, and ICs service requirements.
