Job summary

Family Partners of Minnesota is seeking a compassionate Direct Support Professional to join our Rochester, MN team. You will provide personalized care to individuals with disabilities, helping with daily living tasks and community participation.

You will work to empower clients, support independence, and document progress. The role requires a valid driver’s license, background check, and willingness to obtain CPR/First Aid and medication administration certifications.

Qualifications

• High school diploma or GED required.
• Previous experience in direct care, DSP, or human services preferred.
• Valid driver's license and reliable transportation.
• Ability to pass a background check.
• Strong communication, patience, and problem-solving skills.
• Willingness to obtain required certifications such as CPR/First Aid and medication administration.
• Genuine passion for supporting individuals with disabilities.

Responsibilities

• Provide direct care and daily living support to individuals with intellectual or developmental disabilities.
• Assist clients with personal hygiene, meal preparation, and household tasks.
• Support individuals in community integration and social activities.
• Implement individualized support plans (ISPs) and document progress accurately.
• Administer medications and monitor health needs as directed.
• Foster independence and self-advocacy skills in the individuals you support.
• Communicate effectively with supervisors, families, and care team members.
