Job summary

Rise, Inc. is seeking a Direct Support Professional (DSP) level 2 (Driver) to work with individuals with disabilities in a community-integrated setting.

The role focuses on helping people live purposeful lives through personalized support plans and engaging activities such as art, music, cooking, and community participation. Hours are typically Monday through Friday, 8:30 AM to 4:30 PM, with opportunities for growth and professional development in a supportive, team-based environment.

Qualifications

• Experience or sensitivity working with people with brain injuries and disabilities preferred.
• Must have a valid driver license and be willing to drive a Rise vehicle.
• Able to pass a Motor Vehicle Record (MVR) background check.
• Team player with ability to collaborate in a supportive environment.

Responsibilities

• Directly support people with daily living activities and therapist-designed programs.
• Document Care Plan outcomes and objectives daily.
• Dispense medication per physician orders and maintain documentation.
• Assist Activity Coordinators in setting up activities and encourage participation.
• Inspire personal growth through goal setting and person-centered practices.
• Be a positive role model in appearance, socialization, and teamwork.
• Position involves lifting and transferring adults to/from wheelchairs.
• Complete documentation as needed.
• Provide daily transportation using Rise vans and buses.
• Input information into the myEvolv database.
