Job summary

Rise Incorporated in Wisconsin is hiring a Direct Support Professional (DSP) level 2 (Driver) to work directly with people with disabilities in a community integrated setting, helping them live purposeful lives. DSPs support individuals with customized plans and participate in activities such as art therapy, cooking, and social events.

Qualifications

• Experience working with people with brain injuries and physical disabilities preferred.
• High School diploma required.
• Ability to meet physical demands and use safety equipment.
• Willingness to train and work as part of a team.
• Must pass background checks and have a valid driver license.

Responsibilities

• Assist people with personal cares and daily living activities.
• Document Care Plan outcomes daily.
• Dispense medication per physician orders and maintain records.
• Support activity coordinators in setting up activities.
• Inspire personal growth and implement person-centered practices.
• Model professional behavior and teamwork.
• Assist with lifting and transferring to/from wheelchairs.
• Complete documentation as needed.
• Provide transportation in Rise vehicles.
• Input information into the myEvolv database.
