Merakey is seeking a Drug & Alcohol Case Manager in Philadelphia to provide case management, service planning, advocacy, and resource coordination for up to 30 clients. The role may include on-call duties and requires strong collaboration with health, housing, and community resources.

Qualifications

• High school plus 12 credits of higher education coursework in psychology, addictions studies, social work or related field with specialty or concentration in counseling.
• One to three years case management experience in behavioral health.
• Must be able to work Monday–Friday 7:00am–3:30pm or 7:30am–4:00pm; valid driver's license.

Responsibilities

Join our Drug & Alcohol Program at 5000 Parkside Avenue in Philadelphia, PA, and help individuals build healthier, more stable lives.

As a Drug & Alcohol Case Manager, you'll provide case management, service planning, advocacy, and resource coordination for a caseload of up to 30 consumers, connecting them to the supports they need to achieve their recovery goals. This position may include on-call responsibilities.

Case Management & Client Support

• Serve as primary case manager for assigned clients.
• Conduct regular client contacts and develop person-centered service and safety plans.
• Coordinate care and monitor client progress.
• Assist clients in accessing recovery, medical, housing, employment, transportation, and other community resources.

Documentation & Compliance

• Maintain accurate, timely, and complete client records.
• Complete treatment plans, progress notes, incident reports, and required documentation.
• Track service authorizations and client histories, including hospitalizations and crisis events.
• Ensure compliance with agency, county, state, and federal regulations.

Health & Resource Coordination

• Monitor clients' medical and behavioral health needs.
• Ensure annual physicals and obtain necessary medical documentation.
• Advocate for and connect clients to appropriate services and supports.

Administrative & Professional Responsibilities

• Participate in supervision, team meetings, training, and quality improvement activities.
• Complete all paperwork and reporting requirements on time.
• Perform additional duties as assigned.

Earn $23.62 per hour.
