Job summary

Chapters Health System in Florida is seeking a licensed social worker to provide direct case management for PACE participants. You will screen participants, coordinate care with the interdisciplinary team, and refer to community resources.

Qualifications include a Master's in Social Work, 2 years in health care, 1 year elder care, licensure eligible (LMSW/LISW/LCSW), a valid Florida driver’s license, and a current background check; Alzheimer’s and BLS training are required.

Qualifications

  • Two years’ experience in a health related area desired.
  • One year of elder care experience required.
  • licensure eligible preferred: LMSW, LISW, LCSW
  • Valid Florida Drivers’ License with good record.
  • Automobile Insurance.
  • Active background check as required.

Responsibilities

  • Provide direct social work case management services to PACE participants.
  • Participate in Interdisciplinary Team initial assessments, care planning and re-evaluations.
  • Maintain current case management records with periodic reassessments.
  • Refer participants to community agencies or facilities and advocate for participants.
  • Consult with staff on social, emotional, and cultural factors affecting health and care.

Provides social services support to include participant screening, case management, counseling, and referral.Participate in the Interdisciplinary Team’s initial assessments, care planning and on going re-evaluations of participant’s case.Maintain current written case management records, including periodic reassessments of program participants.Provide individual and family counseling, develop and lead group counseling and activities.Refer participants to appropriate community agencies or facilities, act as a liaison with such organizations and as advocate for participants.Encourage effective relationships amongst staff geared to the building and maintenance of a cohesive team.Consult with and advise staff members as to the relationship of social, emotional and cultural factors to health and medical care, and as to the availability of social services in the community.Participate in program and policy development of the social work component of the program.Represent the company in a positive and professional manner at all times with others, e.g., co-workers, management, vendors, and customers.Consistently represents Hope with integrity and professionalism.Complete all job responsibilities in a thorough and complete manner, utilizing good judgment in all aspects of the job.
