You'll work closely with our nursing staff to develop and implement personalized care plans, ensuring our residents receive the best possible care.
Escort residents to appointments, activities, and social events, providing assistance with mobility and wheelchair transfers as needed
Provide wheelchair assistance and ambulation support to ensure resident safety and mobility
Monitor and report any changes in resident condition or behavior to the nursing staff
Assist with resident care and daily living activities, such as bathing, dressing, and grooming
Collaborate with the care team to develop and implement individualized care plans
Maintain accurate records and documentation of resident interactions and care provided
Current CNA certification
Valid driver's license
Ability to work in a fast-paced environment with changing priorities
