Job summary

Bremerton Trails Post Acute is seeking a CNA to serve as Driver, blending hands‑on caregiving with transportation responsibilities. You will safely transport residents to appointments and outings, assist with transfers, and keep accurate logs while upholding safety and confidentiality.

Qualifications

• Active CNA license (required).
• Valid state driver’s license in good standing.
• High school diploma or equivalent.
• Experience caring for seniors preferred.
• Ability to lift/transfer residents up to 50 lbs. independently (up to 100+ lbs. with assistance).
• Strong communication and customer service skills, with empathy for residents and families.
• Ability to pass an annual TB screening and physical exam.

Responsibilities

• Safely transport residents to and from appointments and destinations, following all facility policies and procedures.
• Assist residents in and out of wheelchairs and gurneys, using proper lifting and transfer techniques.
• Operate and maintain transportation equipment in line with safety guidelines.
• Keep accurate Transportation and Mileage Logs.
• Maintain resident identification records during transfers per policy.
• Build positive relationships with residents, co‑workers, and outside professionals.
• Report any unsafe conditions or equipment issues to your supervisor.
• Maintain strict confidentiality of resident and facility information (HIPAA).
• Participate in ongoing in‑service training, workshops, and seminars.
