Job summary

Bremerton Trails Post Acute is seeking a compassionate Certified Nursing Assistant to provide direct resident care in a homelike setting. You will assist with daily activities, monitor health, and document changes with a strong focus on dignity and safety.

Qualifications

• Comfort working in a 24-hour skilled care environment with residents who may have cognitive or physical impairments.

Responsibilities

• Provide direct care aligned to each resident’s individualized plan of care.
• Assist with bathing, grooming, oral care, dressing, mobility, transferring, and feeding — from set-up help to full physical assistance.
• Support restorative care: ambulation, range‑of‑motion exercises, assistive devices, and proper positioning to preserve function.
• Assist with toileting and elimination needs, including catheter care, following infection control procedures.
• Use safe transfer techniques and equipment, getting help from a coworker when needed.
• Protect resident dignity, rights, and choice in all daily routines and interactions.
• Observe and report changes in behavior, mood, or condition to the charge nurse.
• Document resident care and condition accurately and promptly.
• Communicate regularly with the charge nurse throughout your shift.
• Follow all facility safety, infection control, and emergency procedures; participate in fire/disaster drills.
• Complete 12 hours of annual in‑service training.

Make a real difference in residents' daily lives at Bremerton Trails Post Acute. We're hiring compassionate CNAs to provide hands‑on nursing care that helps residents reach and maintain their highest level of independence in a genuinely homelike environment where dignity and choice come first.

What You’ll Do

• Provide direct care aligned to each resident’s individualized plan of care
• Assist with bathing, grooming, oral care, dressing, mobility, transferring, and feeding — from set‑up help to full physical assistance
• Support restorative care: ambulation, range‑of‑motion exercises, assistive devices, and proper positioning to preserve function
• Assist with toileting and elimination needs, including catheter care, following infection control procedures
• Use safe transfer techniques and equipment, getting help from a coworker when needed
• Protect resident dignity, rights, and choice in all daily routines and interactions
• Observe and report changes in behavior, mood, or condition to the charge nurse
• Use proper body mechanics and equipment (gait belts, mechanical lifts) to safely move residents ranging from under 80 to over 300 lbs
• Document resident care and condition accurately and promptly
• Communicate regularly with the charge nurse throughout your shift
• Follow all facility safety, infection control, and emergency procedures; participate in fire/disaster drills
• Complete 12 hours of annual in‑service training
