Job summary

Restore Hyper Wellness in Price, UT is seeking a Direct Support Professional to accompany individuals and staff to activities, appointments and community events. When not driving, you will work in homes and complete assigned tasks.

You will collaborate with the Chrysalis Team to ensure safety, respect and mentorship while helping participants enjoy meaningful daily activities and grow their quality of life.

Qualifications

• Criminal Background Check is required.
• Must be 21 years of age or older.
• Must meet and stay in compliance with driving requirements.
• Capable of communicating well in both verbal and written form.
• Able to complete all assigned program documentation accurately and within the allotted time frames.
• Have good judgment and able to handle crisis situations.
• Have a positive attitude.

Direct Support Professionals Responsibilities

Direct Support Professionals in the day program work with the individuals in workplace setting and in the community. Your main job is to create opportunities for the individuals to increase their quality of life.
