Job summary

Chrysalis of Ogden, UT is seeking Direct Support Professionals for the day program. You will work with individuals in workplace training settings to ensure safety, respect, mentorship and fun, acting as a teacher, mentor and friend. Sites include Layton, West Haven and Ogden.

You must pass a Criminal Background Check, be willing to drive a company vehicle, and hold a Utah driver's license. Schedule is 7am–4:30pm Monday to Friday, with major holidays closed.

Qualifications

• Be able to work 7am-4:30pm Monday through Friday and closed on major holidays.
• Must pass Criminal Background Check.
• Must be willing to drive a company vehicle and be an approved driver.
• Must be at least 21 with clean driving record or up to 2 incidents in 3 years if older; valid Utah license.
• Capable of clear verbal and written communication.
• Accurate completion of program documentation on time.
• Good judgment and ability to handle crisis situations.
• Maintain a positive attitude.

The Direct Support Professional in the day program is the lifeblood of the Chrysalis Team. You work hand in hand with the individuals in a workplace/training setting to ensure they are safe, respected, mentored and having fun. Direct Support Professionals Responsibilities

Direct Support Professionals in the day program work with the individuals in workplace setting and in the community. Your main job is to create opportunities for the individuals to increase their quality of life.

Position Qualifications

• Be able to work between the hours of 7am-4:30pm Monday through Friday and closed all major holidays.
• Must be able to pass a Criminal Background Check
• Must be willingto drive a company vehicle to transport individuals.
• Must be an approved company driver in order to drive a company vehicle:
  • Must be at least 21 years or older with:
    • If 21 years old: No moving violations and or accidents.
    • If 22 years old or older: Must have no more than 2 moving violations or accidents in a 3-year period.
    • A valid drivers license in the state of Utah or be able to provide a current driving record
• Capable of communicating well in both verbal and written form
• Able to complete all assigned program documentation accurately and within the allotted time frames
• Have good judgment and able to handle crisis situations
• Have a positive attitude
