Job summary

Chrysalis of Ogden, UT is seeking a Direct Support Professional to work in our day program, partnering with individuals to ensure safety, respect, mentorship and engagement in activities. The role emphasizes acting as a teacher, mentor and friend while supporting community outings and on-site activities.

Qualifications

• Be able to work between 7am-4:30pm Monday through Friday and closed all major holidays.
• Must be able to pass a Criminal Background Check.
• Must be willing to drive a company vehicle to transport individuals.
• Must be an approved company driver and hold a valid Utah driver’s license with a clean driving record.
• Capable of communicating well in verbal and written form.
• Able to complete all assigned program documentation accurately and within the allotted time frames.
• Have good judgment and a positive attitude.

Team Player

The Direct Support Professional in the day program is the lifeblood of the Chrysalis Team. You work hand in hand with the individuals in a workplace/training setting to ensure they are safe, respected, mentored and having fun. You get paid to go to movies, go fishing, go bowling, play games, and to be a teacher, a mentor and a friend. Direct Support Professionals TRULY have an opportunity to Make A Difference Daily!

Work sites can include: Layton, West Haven, and Ogden area

Direct Support Professionals Responsibilities

Direct Support Professionals in the day program work with the individuals in workplace setting and in the community. Your main job is to create opportunities for the individuals to increase their quality of life.

Position Qualifications

• Be able to work between the hours of 7am-4:30pm Monday through Friday and closed all major holidays.
• Must be able to pass a Criminal Background Check
• Must be willing to drive a company vehicle to transport individuals.
• Must be an approved company driver in order to drive a company vehicle:
  • Must be at least 21 years or older with:
  • If 21 years old: No moving violations and/or accidents.
  • If 22 years old or older: Must have no more than 2 moving violations or accidents in a 3-year period.
  • A valid drivers license in the state of Utah or be able to provide a current driving record
• Capable of communicating well in both verbal and written form
• Able to complete all assigned program documentation accurately and within the allotted time frames
• Have good judgment and able to handle crisis situations
• Have a positive attitude
