Job summary

A Place At Home is seeking a DODD Care Coordinator to assess the needs of individuals with developmental disabilities and develop personalized service plans. You will coordinate support services from multiple providers and advocate for the well-being of clients within state guidelines.

The role involves working with families, maintaining documentation, and ensuring service quality through ISP implementation and regular provider communication.

Qualifications

• Bachelor's degree or equivalent in social work, human services, psychology, or a related field.
• Strong communication and interpersonal skills to engage clients, families, and providers.
• Excellent organizational and time-management abilities to manage complex caseloads.
• Ability to work independently and collaborate with a multidisciplinary team.
• Understanding of state regulations and funding sources related to developmental disabilities.

Responsibilities

• Assess individuals' needs in daily living, medical care, communication, social interaction, and employment.
• Develop and manage personalized Individual Service Plans (ISPs) with individuals and families.
• Coordinate services from residential facilities, day programs, therapy providers, and transportation.
• Maintain regular communication with providers to ensure quality of care per the ISP.
• Engage families or guardians in information sharing and decision-making.
• Advocate for individuals to access services and navigate systems.
• Document assessments, ISP details, progress notes, and communications.
