Job summary

Help at Home is seeking a Developmental Disability Professional (DDP) to empower individuals with developmental disabilities in Atlanta. The role involves leading person-centered planning, developing ISP outcomes, and supervising Direct Support Professionals to deliver tailored supports.

You will monitor progress, revise plans as needed, and collaborate withCase Managers to ensure compliant, high-quality services that promote independence and safety for clients at home.

Qualifications

• BA/BS Degree in social work, psychology, human services or related field.
• One (1) year of experience in the field of developmental disabilities.
• Supervisory experience preferred.

Responsibilities

• Actively participates in person centered planning and ISP development and implementation.
• Design and implement support and teaching strategies to help consumers reach ISP outcomes.
• Provide supervision and training to Direct Support Professionals on PCP/ISP implementation.
• Monitor PCP/ISP progress and adjust outcomes based on data and feedback.
• Coordinate with Case Manager and interdisciplinary team on reports and plans.
• Ensure services are provided per PCP/ISP and required regulations.
