Qualifications

• Bachelor’s degree in social work or closely related field.
• 2+ years of case management experience involving adults, juveniles, and families.
• Masters degree in social work or closely related field preferred for higher level; licensure as applicable.

Responsibilities

• Coordinate diversion program services with internal and external providers.
• Hold multi-disciplinary meetings and prepare case management plans.
• Maintain client records and document contacts and progress.
• Advise families on educational options and restorative justice processes.
• Collect data and report program statistics and outcomes.
