HCT Healthcare in California seeks an experienced nursing professional to serve as the Manager of Continuity of Care for a 270+ bed hospital. Lead the hospital case management department and drive standard processes, educate staff, and strengthen accountability for patient outcomes.

Responsibilities include discharge planning, utilization management, length of stay reduction, and building strong relationships with physicians and the care team.

Qualifications

• BSN degree required.
• Minimum 5 years progressive case management leadership experience in an acute care hospital.
• Current CA RN license required.
• BLS through AHA required or completed during onboarding.
