Job summary

Owensboro Health Regional Hospital is seeking an experienced discharge planning RN to facilitate cost-effective, high-quality transitions of care. You will collaborate with the care team to assess needs and develop comprehensive discharge plans for post-hospital care.

The role requires a Bachelor's degree in Nursing and RN licensure in Kentucky, with at least 1 year of relevant experience preferred. You will coordinate with patients, families, and clinical staff to ensure timely, accurate

Responsibilities

Coordinates and manages discharge planning activities.
Assesses medical, financial, emotional status and support system to develop post-hospital care plans.
Collaborates with patients, families, and the healthcare team including physicians and nurses.
Provides information and education on post-hospital care options and resources.
Facilitates referrals to external facilities and community services.
Ensures all paperwork and documentation are completed accurately and timely.
Monitors progress and updates discharge plans as needed.
Communicates complex issues to Manager/Director/Physician Advisor.
Trains new staff, students, or interns as needed.
