Qualifications

• Bachelor's degree in Nursing required upon hire.
• RN - Licensed as a Registered Nurse; Kentucky license required upon hire.
• Minimum 1 year of relevant experience preferred.

Responsibilities

• Coordinates and manages discharge planning activities.
• Performs comprehensive assessments to develop discharge plans for post-hospital care.
• Collaborates with patients, families and healthcare team members.
• Provides education on post-hospital care options and resources.
• Facilitates referrals to external facilities or community services.
• Ensures paperwork and documentation are completed accurately and timely.
• Monitors progress and updates discharge plans as needed.
• Communicates with managers and physicians on complex discharge issues.
• Trains new staff, students or interns as needed.
