Job summary

American Advanced Management is seeking a full-time Social Worker to plan, organize, and implement discharge planning and post-acute care services. The role focuses on patient and family engagement, coordinating community resources, and ensuring continuity of care after discharge.

The position requires MSW or equivalent with 1–3 years of related experience; must be able to comply with safety and company policies. Day shift, 8:00 AM to 4:30 PM, in Stockton, CA.

Qualifications

• Education: Graduate of an accredited college or university with a baccalaureate or advanced degree in social work. MSW preferred.
• Experience: 1-3 years in related field or similar situation preferred.

Responsibilities

• Plan, organize, and implement social work, discharge planning, and post-acute care services for patients and families.
• Use patient and family engagement to align with goals and preferences; connect with community resources.
• Identify patients at risk of abuse/neglect and intervene per California reporting processes.
• Coordinate services with Resource & Utilization Management to ensure cost-efficient care and continuity post-discharge.
