Role Description

The Social Worker is a full-time, on-site role based in Fargo, ND, supporting residents and families. Day-to-day responsibilities include completing psychosocial assessments, developing person-centered care plans, and coordinating services to address emotional, social, and practical needs. The Social Worker will provide counseling, advocacy, and crisis intervention, help residents transition between care settings, and facilitate discharge planning when appropriate. The role collaborates closely with nursing, therapy, activities, and other care teams, communicates regularly with families, and maintains accurate documentation in accordance with regulatory standards. The Social Worker also participates in interdisciplinary care conferences, supports resident rights, and contributes to quality improvement initiatives.
