Job summary

Centinela Hospital Medical Center in Inglewood, part of Prime Healthcare, is seeking a Per Diem Case Management/Discharge Planning coordinator to support discharge planning under the direction of the case manager.

Responsibilities include coordinating discharge plans, assisting with referrals and authorizations, and maintaining Medicaid submission forms; interview patients/families to develop safe, patient-centered plans.

Qualifications

• High School Diploma required.
• Knowledge of Discharge Planning and Case Management terminology.
• Experience with basic computer skills.

Responsibilities

• Coordinate the activities of the Case Management Department under the direction of the Case Manager.
• Assist with development and implementation of discharge plans.
• Maintain Medicaid state submission forms for reviewer and file per procedure.
• Interview patients/families to develop a safe patient-focused Discharge Plan.
