Qualifications

• Bachelor’s Degree in Social Work or related social field with experience listed below, or Licensed Vocational Nurse with experience listed below.
• For BSW: One year experience in a healthcare setting or field placement in a healthcare setting preferred.
• For Bachelor’s in a social-related field: Recent experience of more than three years in case management and/or utilization management preferred.
• For LVN: Recent experience of more than three years in case management and/or utilization management preferred.

Responsibilities

• Discharge planning interventions with patients/families under the direction of Case Managers and Social Workers.
• Acts as liaison to internal/external case managers and payors around discharge planning.
• Perform multiple activities to support patient movement and transitions from inpatient hospital setting.
