Job summary

ProMedica's Flower Hospital is seeking a Social Worker/Discharge Planning Coordinator to support patients and families, assess psychosocial risks, and coordinate discharge planning for high-risk individuals.

You will connect patients to community resources, advocate during hospitalization, and address psychosocial concerns such as mental health or guardianship, ensuring continuity of care across pre- and post-acute settings.

Qualifications

• Education: BSW or higher; will consider a limited license SW on individual basis.
• Knowledge of regulations, requirements, and community resources; ability to function in a critical care environment.
• License: L.S.W - Licensed Social Worker

Responsibilities

• Coordinate discharge plan of care for high-risk patients.
• Assess psychosocial risk factors for patients and caregivers.
• Provide emotional, social, and financial support by connecting to community resources.
• Advocate for patients during hospitalization and address psychosocial concerns.
