Job summary

ProMedica Toledo Hospital is seeking a Social Worker/Discharge Planning Coordinator for the Flower Hospital Care Navigation team. You will assess psychosocial risks and coordinate discharge plans for high-risk patients, connecting them to community resources and providing emotional and financial support.

The role requires a BSW or higher (LSW considered) with strong written and verbal communication, critical thinking, and excellent interpersonal skills.

• BSW or higher degree; may consider limited license SW on an individual basis.

Responsibilities

• Assess psychosocial risk factors for patients and caregivers.
• Coordinate and facilitate discharge planning for high-risk patients.
• Provide emotional, social, and financial support by linking to community resources.
• Advocate for patients during hospitalization and ensure continuity of care.
• Address psychosocial concerns including substance use, mental health, and guardianship.
