Qualifications

Responsibilities

• Perform psycho-social evaluations of patient and family systems.
• Develop discharge plans with patient/family and interdisciplinary team.
• Complete functional psychosocial assessments within 24 hours.
• Coordinate services and referrals to ensure safe discharge.
• Collaborate with physicians and team to develop comprehensive discharge plans.
• Provide counseling and crisis intervention as needed.
