Job summary

Nebraska Methodist Health System in Council Bluffs, IA, is seeking a Social Worker to support discharge planning and psychosocial assessment for patients and families. Qualifications

• Education: Bachelor's Degree in Social Work or Human Services related area.
• Master’s degree preferred.
• CSW certification preferred.

Responsibilities

• Complete psycho-social evaluation within 24 working hours of referral.
• Develop discharge plan with patient/family and interdisciplinary team.
• Coordinate post-hospital services and community resources.
• Document discharge needs and barriers to ensure safe transition.
• Assist with advance directives, guardianship, and crisis referral.
