Job summary

Qualifications

• Bachelor’s Degree in Social Work or Human Services related area required.
• Master’s degree preferred.
• Certified Social Worker (CSW) preferred.
• Minimum 1-2 years relevant discharge planning or counseling experience required.

Responsibilities

• Psychosocial evaluation of patient and family system.
• Develop discharge plan with patient/family and interdisciplinary team.
• Complete psychosocial assessment within 24 working hours of referral.
• Provide referral to appropriate community resources and services.
• Collaborate with physician and interdisciplinary team to develop proactive discharge plans.
• Communicate barriers to discharge with patient/family, physician and team.
