Job summary

Largo Nursing & Rehabilitation Center in Largo, MD is seeking a Discharge Planning & Social Work Assistant to ensure smooth transitions from our facility back into the community. You’ll collaborate with the Director of Discharge Planning & Social Work to address emotional, social, and practical needs of patients and families, guiding access to resources for a safe discharge.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, criminal justice, or other human services related field.
• Experience in post-acute social work, discharge planning, and patient relations.
• Proficient with computer programs including Microsoft Office Suite (Word, Excel, etc.)
• Excellent communication and interpersonal skills.
• Highly organized and detail oriented.
• Ability to work independently or as part of a team.

Responsibilities

• Advocate for residents’ rights and preferences, ensuring discharge plans align with goals for care and community integration.
• Collaborate with interdisciplinary teams to develop and facilitate individualized discharge plans.
• Coordinate post-discharge services, including home health care, community resources, and follow-up appointments.
• Assist patients in accessing and utilizing community resources, such as transportation services and support groups.
• Provide emotional support and companionship to patients, particularly those experiencing loneliness or anxiety.
• Other tasks to support the successful operation of the healthcare center.
