Job summary

Responsibilities

• Coordinate patient discharge including supplies, transportation, and services to ensure a safe transition.
• Schedule discharge meetings and coordinate home visits, home care, and therapy as needed.
• Direct five-day discharge planning meetings for newly admitted residents.
• Assess patient support systems and home situation to inform discharge planning.
• Facilitate bed management to maximize availability of transitional and extended care services.
• Meet with residents to address social issues and coordinate care.
• Conduct customer satisfaction surveys.
