Qualifications

• Master's Degree in Social Work required.
• Licensed Clinical Social Worker (LCSW) credential.
• Knowledge of discharge planning processes and community resources.
• Understanding of Medicare/Medicaid and third-party payor systems.
• Commitment to continuing education and up-to-date knowledge.

Responsibilities

• Assess patient’s needs (physical, psychosocial, cultural, spiritual) and coordinate referrals.
• Facilitate discharge planning with patient, families, and treatment team; document actions.
• Assess emotional, social, and financial needs; set up community resources.
• Assess/reassess clinical and psychosocial status per Case Management referral.
• Collaborate with interdisciplinary team to develop and implement plan of care.
• Provide support to patients and families coping with changing medical conditions.
